Eagle Mountain Lake Cleanup
Please note, although you can register to participate in the cleanup through this web site, only a parent or guardian is allowed to submit a release for their child. If you are registering for a large group and need additional liability forms, please download the release form here or contact our Eagle Mountain Office, 817-237-8585 so we can mail them to you. Options for returning them are by fax, 817-237-8563, by mail (TRWD, Attn: Cleanup Coordinator, 10201 North Shore Drive, Fort Worth, TX 76135) or you can bring them with you when you collect your cleanup gear.
LIABILITY RELEASE As a participant, or parent or guardian of a participant in the Watershed Awareness Program sponsored by Tarrant Regional Water District and the participating sponsors and co-sponsors, I, undersigned, hereby release, discharge, and agree to hold harmless the Tarrant Regional Water District, all sponsors & co-sponsors, their agents, employees, officers, and successors from all liability, claims or actions which I, my heirs, executors, administrators, or assigns may have or claim to have against any of them arising from any personal injuries or other claims connected therewith, whether known or unknown, or injuries to other persons or to property caused by or arising out of any actions I might take relating to my activities while participating in the above program. If the participant is a minor, I, as parent or guardian of the participant, further authorize the Tarrant Regional Water District and the participating sponsors, co-sponsors, and employees of theses organizations to obtain emergency medical treatment for the participant, should an apparent need for this treatment arise.
